I am Adelia Caliva a registered nurse currently working as a clinical nurse mentor for the Aurum institute an NGO in Ngaka Modiri Molema, North West Province. I have completed my degree in occupational health nursing in December 2020, awaiting graduation at Tshwane University of Technology, where I will receive my qualifications certificate. I have been working in primary health care facilities since community service year 2012 up to date. I have been groomed in the Primary health care facilities, NIMART certified, have a dispensing licence, IMCI, TB, STI, Adult Primary Care and so forth. Currently I support government facilities with strengthening of implementation of guidelines, training, mentoring and clinical work by roving. I work hand in hand with clinicians, community health care workers, counsellors, tracers and other stakeholders towards achieving the governmental targets set. My daily duties are at work is consulting patients using the according to the standard operating guidelines for management of chronic conditions including HIV, TB in the facilities (primary health care) I support which are 6 located in the villages around Zeerust. We have data capturers in facilities who provide us with data regarding our HIV/TB clients and progress which guides me on planning for my work. I also perform HIV testing, plan outreaches and HIV campaigns with assistance of community health workers, my manager and the department of health. In the facilities I support I work with a HIV counsellor, tracer and data capturer from the same company whom I supervise, they report to me and I report to the facility manager and local area manager daily of work done after work. Weekly work plans are created and sent to my supervisor on a weekly basis. Rendering of health education and promotion to clients and community as a group, and individually according to abnormalities found or problem presented or diagnosis I consult all clients that are HIV/TB positive, counsel them on the importance of adherence and compliance, initiate them on treatment and monitor them as we capture them on our data system called http://www.******.***. I collect baseline and monitoring bloods according to the guidelines, interpret results to clients and manage according to the results. Index testing done to clients children, partners by finding out from the HIV or TB positive client about their partners and children. I advise clients to disclose to partners and relatives. Switching HIV clients from 1st line regimen to 2nd line of those with virological failure. Nutritional assessment. Drugs or medicine control Monitoring of emergency trolley Management of side effects of ARVs and opportunistic infections such as STIs, pneumonia, skin conditions, upper respiratory tract infections. Referral of complicated cases to specialist Dr’s to hospitals. Tracing of HIV and TB clients nit yet on ARVs, enrolling them on treatment. We keep all information confidential. I am given targets by the company for indicators per facility for initiating, decanting, viral load monitoring, provider initiated counselling and testing, and if targets are not met I give reasons and create remedial actions. Whatever is done in the facility and indicators not being achieved I communicate with the registered nurses and operational manager in the specific facilities. I perform file audits and inform the other nurses about gaps, also perform mentoring of nurses and onsite training with regards to management of clients. I trace telephonically and via address clients whose blood results are abnormal or defaulting treatment together with chws. Drop off of packaged medications to clients who are not able to attend the clinic for specific reasons. Liaising with other stakeholders such as CHAPS, CAREWORKS, outreach team leaders from DoH. Attend biweekly reviews were performs of sub-district is reflected, discuss challenges and solutions for implementation created. Reports done, daily, weekly and monthly that I email to my supervisor who is my local area manager I might not have mining industry experience but I have community and primary health care experience for 9 years, my studies and practicals for occupational health nursing these past two years has build a foundation to work in any industry, the holistic health of the community and workers is vital, I am aware of what risk assessments to do, how it is done at work, and how to manage risks found in the working environment. I know of occupational and non-occupational conditions or diseases. With orientation to the mining industry I will definitely grasp the working environment and adjust easily and my passion for community work also plays a major role. I am hard working, flexible, passionate, have a zeal for learning, challenges, humble, respectful. I believe that my willingness to learn and sharing the knowledge that I’ve gained through my past working experience and studies can only serve benefit to the company. I am the best candidate because I am young, energetic, I have worked with the community and I love the work I do and want to grow as well.
